General Info for Parents

Submit a separate form for each of your children participating.
You MUST register your child or they will not have individual photos taken.

IMPORTANT : By submitting this form and registering your child for photos, you're giving Genesis Photography permission to photograph your child at their center and place those photos in a private password-protected online gallery.
